The film is based on the book by Matt Baglio, inspired by real events, that follows the story of an American parish priest, Father Gary Thomas (Donoghue), who travels to Rome to train as an exorcist at a Vatican-associated university. As the young priest begins to carry out exorcisms, under the guidance of a seasoned Italian exorcist (played by Hopkins), he is forced to alter the way in which he perceives the spiritual world and the conflict between good and evil.
